Home and community-based organizations are privately funded business but many receive a wide range of government-provided or government-funded programs and services that are intended to support and assist individuals and families in need including: at high risk for Emergency Department use, hospitalization/hospital readmission or nursing home placement, complications with dementia disorders, behavioral health needs and substance abuse disorders, and homelessness or at risk of being unhoused.
